In the preschool room, children were observed watching a Mickey Mouse cartoon during lunch from the tablet. . Noted: this was corrected during the inspection when the tablet was turned off.

The large playground was littered with discarded water cups on the ground throughout the playground. Children were observed picking up the trash and throwing it over the fence. It is also noted that there were discarded garbage bags in front of a shed that stores outdoor toys that children are able to access. Noted: this was corrected during the inspection when the trash was picked up and discarded.

The children's product form has not been re-certified.

The bottom of the metal door stopper had a screw that was protruding, creating a hazard. I observed on corner of the small playground water hose, metal frame, tarp, cot and trash. Ants were observed on the corner of the small playground on a tarp. Note: During inspection caregiver removed the screw.

Hand soap and paper towels were not independently accessible to children at the Preschool hand sinks. This was corrected at inspection when a soap dispenser was provided and hand towels were stocked.

The loose-fill under the play structures on the Pre-K playground and the dual slides in the Toddler playground were less than six-inches.

Three of ten children's records audited did not have current immunizations and two did not have current health statements.

Upon review of video footage provided, it was observed that an infant was placed in their crib awake and remained awake in the crib for over 30 minutes.

At time of inspection a diaper changing station in a preschool class was observed to have debris. At time of inspection multiple handwashing sinks were observed to be stained. At time of inspection a childrens restroom door was observed to have a hole at the bottom of the door.

At time of insepction transportation training for staff who provides service expired on 5.28.24.

Based on information gathered, a child was left on the playground unsupervised.

Based on information gathered, caregiver did not interact with child in a positive manner, when caregiver lifted child by one arm and pulled child to the other side of caregiver.

A caregiver failed to conduct a face to name check on children entering from the playground to the school age classroom, resulting in a child being outside unsupervised for approximately nine minutes.

Three bottles of disinfecting solution were observed in the school age classroom, at the children's reach. Note: Corrected at inspection when staff moved the disinfecting solution to the top of the cabinet, no longer creating a hazard.

Based on the information gathered, a child was able to open the chain link fence gate, and child crossed the parking lot and child was found on the hill in front of the center parking lot, child was left unsupervised for 2 minutes.

Director lacked training in transportation.

Based on information gathered, caregiver did not interact with child in a positive manner, when caregiver began to roughly pick up child by one arm, then continued to pick up child under other arm and moved child to cot.

Based on the investigation conducted, there is sufficient evidence to state that a caregiver did not interact positively with a child in care when the caregiver pulled the chair while the child was sitting in it, resulting in the child falling.

Two children's medication was observed to be expired.

Operation failed to report Covid-19 incidents to Licensing within 48 hours of confirmed diagnosis.

Based on the information gathered, a child was left unsupervised on the playground for about 25 minutes.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required.

In the Pre-K classroom the caregiver stated that there were 14 children and there were 15 children counted. Then another child was added to the group making it 16 children in the classroom without caregiver awareness. In the Pre-K 3 classroom the caregiver stated thAt there were 14 children and there were 15 children counted.
