During the investigation, it was determined that the first aid certificates sent to CCR were obtained without taking the first aid course.

During the investigation, it was found that two of the staff members did not have first aid certifications.

During an investigation, it was determined the operation's transportation policy did not include what steps the operation would take when a child is not present during afterschool pick up. The policy also did not include parent notification of transportation pick up.

The electronic child safety alarm was observed not functioning in one of the operation's vans used to transport the children in care.

1 out of 10 child immunization records were not current at the time of inspection.

I reviewed 10 records and 2 children did not have up to date immunizations at the time of inspection.

It was determined through an investigation that proper supervision was not practiced when a child was able to exit the building without a caregiver present/being aware.

The operation failed to submit the number of employees that have left their employment in 2023 by January 15th.

The operation failed to safely store the child's formula by misplacing it.

After parent notified the director/owner of the child's needs, it was not ensured that the caregivers were following infant care instructions.

The AM and PM caregivers were not effectively communicating information which resulted in the child being sent home in LUVS diapers on multiple occasions.

A caregiver was obserrved to be sitting a doorway supervising two separate classrooms during naptime.

Children are receiving injuries (biting) due to improper supervision.

The parent was not notified of the bite the child received while in care.

The Director does not have proof of attendance records that documents time and hours present at the operation.

Infants were restricted to their cribs due to being combined in the classroom with older children for longer than fifteen minutes.

Four infants were being cared for with children over eighteen months old in the first 45 minutes of opening the operation.

There was only one caregiver observed to be caring for 16 children ranging from 3 months to 4 years old in the infant room.

Four out of five bottles in the infant room were not labeled by name as required. This was corrected at inspection when the names were added to the bottles.

There were six infants ages two months to thirteen months with one caregiver. The maximum amount of infants for this age group is four infants per one caregiver, therefore this group was not compliant for proper ratio. This was corrected at inspection when another caregiver arrived to bring the ratio into compliance.

There was one outlet in the toddler room that was not covered or protected. This was corrected at inspection when an electrical outlet cover was provided.

The operation did not have the required Child Product Safety Commission Form. This was corrected at inspection when the director downloaded the form, signed it, and posted it.

During naptime, it was observed that a four month old infant was combined in the same room with older children up to four years in age. Minimum standards state that children under eighteen months of age are only allowed to be combined with older children as long as the oldest child is less than eighteen months older than the youngest child, therefore the operation was not in compliance. This was corrected when another caregiver arrived, and the infants were separated from the older children.

Three out of eight children's records evaluated were missing the required emergency contact information.

Based on information gathered during the course of the investigation, caregiver did not give child his medication at 10:30 am as indicated on the medication log.
