During the inspection 3 of the 3 staff files reviewed were missing annual SIDS training.

During the review of the directors trainings, it was found she was missing the annual SIDS training.

During the inspection a 3 month old infant was being held by the caregiver in charge. This was corrected immediately by the director stepping in and having the caregiver bring the infant to the front office.

One of ten children's records did not include proof of up-to-date immunization records.

Three infant feeding form was not available for review.

Three out of ten children's files reviewed did not have a health statement available for review.

Seven out of Seven staff files were missing training hours for the required annual trainings.

Two out of ten reviewed files were missing non-parent emergency contacts.

One of six caregivers had not renewed CPR/First Aid certification which had expired in March/2024.

Two of eight children's records did not include proof of up-to-date immunization records.

A new employee did not have First Aid certification and had unsupervised access to children in care.

Of the six caregiver files:. two caregivers had verification of only nine of the required twenty- four hours of annual training. One caregiver had only seventeen of the required twenty-four hours and the director did not have verification of the full thirty required hours needed.

A sleeping infant was observed with a bib fastened around his neck. NOTE: This was corrected as the caregiver removed the bib immediately.

During an investigation, it was determined that the operation was offering and providing transportation services to children in care without any prior notification to Child Care Regulation, as required.

The operation did not submit the required number of employees lost during the year 2023, by the due date of 1/15/24.

The most recent health inspection was conducted in March/2022.

One of four children's files did not have updated shot records. Two of four children's files did not have their health care statement included.

Documentation of a daily inspection from 6.29.22 was unavailable at inspection. NOTE: This was corrected when a walkthrough was conducted.

No documentation of the carbon monoxide, fire extinguishers, and smoke detector. NOTE: This was corrected at inspection.

Based on the information provided by the caregiver, hand sanitizer was used to wash hands before lunch. NOTE: This was corrected when the children were taken to the restroom down the hall.

One of the six children records did not have a current immunization records available for review.

Safety Product was last reviewed on 1.12.21. NOTE: This was corrected when director viewed all current and past recalls through the CPSC?s Internet website.

Two wooden boards from the fence on the playground were observed missing posing a hazard. NOTE: Corrected at inspection when the boards were installed.

Two caregivers were observed working in the classrooms supervising children without receiving a letter of eligibility. NOTE: Corrected at inspection when staff left the facility and BGC were submitted.

Four children under the age of the two years old were observed watching TV before lunch.

Operation did have not have monthly infant feeding schedule on file for a three month old infant.

Two children's files were missing the address for their emergency contact.
