A seven month old infant was sleeping in the crib wearing a swaddle-up grown. Note, the hands were free, but the wrap was around the body. Corrected at inspection when the gown was removed.

During the course of the investigation, it was determined that a caregiver for toddlers failed to wash their hands after each diaper change.

During the course of the investigation, it was determined that a toddler caregiver lacked good judgment when they picked up toddlers using only one hand.

During the course of the investigation, it was found that a toddler caregiver did not wash the children's hands following diaper changes.

The director did not have the health and safety trainings available for review at the time of inspection.

The caregiver in the preschool classroom gave the incorrect number of children in care in the classroom. Caregiver stated there were eleven children when there were twelve children in the classroom.

No incident report was created for the incident or shared with the parent.

A child was left alone on the playground for approximately twenty-five minutes while their class returned to the classroom.

At time of inspection a PreK caregiver stated to have nine children when the class had seven.

During the inspection, debris was observed in an infant diaper changing station. Additionally, classrooms were found to have debris on the floors and a Pre-K classroom restroom was also noted to have debris stains on the walls.

It was found during a DFPS investigation that a staff member was confirmed to have used profane language directed at children in care.

It is found that a child with known special needs was not supervised to the level of care that they are known to need, which allowed them to exit the operation unsupervised and proceed to the edge of the parking lot of the operation. The child was immediately returned to care unharmed after being stopped by the parent of another child.

Equipment that had been recalled in 2017 was being utilized in the infant classroom.

A non-routine situation occurred which placed a child at risk of injury and was not reported to licensing.

A caregiver was observed grabbing and flipping children in an aggressive manner during nap time.

There were two restrooms that had trash cans without lids on them. The trash cans had dirty diapers thrown into them.

There were two infant bottles located in the fridge that were not labeled with the child's first name and last initial. Note: This was corrected at inspection when the caregiver in the classroom labeled the infant's bottles with the child's first name and last initial.

There were 7 infant feeding schedules that had not been updated for the month of February.

A child was left unattended on the playground for 18 minutes.
