The caregiver in the pre-k classroom did not know the number of children in her care.

The school age caregiver was squirting children with a water gun during water play. Then a child grabbed the water gun from the caregiver and sprayed the children. The child also sprayed me with the water gun as I was entering the fence.

The pre-k classroom was found with the lights off making the back of the room dark during naptime.

At time of inspection, infant bottles were observed to not be labeled. NOTE: This was corrected at inspection.

Cots were not assigned to children. Cots were not labeled with children's names nor were they labeled with numbers. Corresponding child/mat assignment sheets did not exist in any classrooms.

Children 14-months and younger were observed having screen time. This was corrected at inspection when a discussion was had with the director regarding screen time and the tablet was turned off.

A Pre-K child was observed leaving the bathroom, walking past the hand washing sink without washing their hands, and going to their cot. This was corrected at inspection when a discussion was had with the caregiver and the child was directed to wash their hands.

Diapers were observed in an uncovered trash can accessible to children.

An infant was observed walking around with a bottle.

A slide on the playground had a ladder that was cracked and broken creating a hazard.

Changing area was not sanitized after a child was diaper changed. Note: This was corrected at inspection when the caregiver was redirected to sanitize the diaper changing area during inspection.

A child's hands were not washed after a diaper change. Note: This was corrected at inspection when the child's hands were washed.

Based on observations, a child 18 months older than the youngest infant was observed in the infant classroom.

Based on the investigation conducted, there is sufficient evidence to state that a staff member inappropriately disciplined their own child at the operation.

The four year old classroom was observed out of ratio as there were 22 four years old to one caregiver. The correct ratio is 1 caregiver to 18 four year olds. Note: This was corrected at inspection as the Director stepped in to the classroom.

The storage for the school agers belongings was not labeled with their child's name, a photograph of the child, or other symbol the child recognizes as his own. Note: This was corrected at inspection as the storage space was labeled with the child's names.

A caregiver's personal bag was observed accessible to the school aged children in care. Note: This was corrected at inspection when the bag was moved to a high shelf.

Three out of the six staff record's reviewed did not include proof of education.

The electronic safety alarm was not observed installed in one of the operation's vehicle used to transport children.

The use zone of the swing and slide were observed overlapping.

The emergency preparedness plan did not include the required topics.

Televisions observed in classrooms where children are cared for in were not mounted.

The operation did not have a current sanitation inspection conducted.

The operation did not have a current fire inspection.
