During the walkthrough, four children were observed unsupervised in a classroom.

The annual fire drill log did not have fire drills logged for April, May, June, or July of 2025.

During the inspection, medication was being stored: for one child no longer enrolled, for one child no longer being administered, and two expired medications.

The changing table pads in the toddler rooms had cloth coverings, which were not able to be sanitized in between diaper changes.

During the walkthrough, garbage cans in multiple classrooms were observed with no lids, leaving garbage accessible to the children.

During the staff file review, it was found that two caregivers did not have the in-person portion of the CPR training. One caregiver had an expired CPR card.

During the walkthrough, there were cleaning products and other chemicals in cabinets of multiple classrooms accessible to children.

The operation has not provided proof of liability insurance coverage by anniversary date, which was 9/23/25.

An individual with a pending background check was present in a classroom with 13 children. A caregiver with a eligible background check present, additionally. Noted: The individual left the premises during the inspection.

A caregiver when asked did not know the correct number of children in her classroom.

The operation failed to report the number of employees that left their employment by the required January 15th deadline.

Caregivers failed supervise appropriately when a 2 year old child was found outside the operation. Caregivers were unaware that the child had left the facility until the child was returned by a neighbor.

Neglect occurred when a two year old child was found outside of the operation. The child was unaccounted for approximately 7-8 minutes and was found by a neighbor. The child was at risk of serious harm.

A volunteer who was left alone with children did not have FBI check eligibility.
