Operation video footage demonstrated an operation staff member push a child down on their head making child fall/sit on their cot. Operation video footage demonstrated a staff member pulling the mat from beneath a child, causing the child to slip off. Operation video footage demonstrated an operation staff member grab a child by one ankle and one wrist and flipping child around on their cot. Operation video footage demonstrated an operation staff member shake pants off a child sleeping on a cot to put on new ones.

When reviewing staff files one staff member out of ten was missing training certificates to review.

In the handbook or application there was no signed verification that the parents were aware of their parent rights.

2 classrooms were missing emergency evacuation maps. * Corrected at inspection.

Documentation for a gas leak test that was completed in the last two years was not avalible for review.

Two out of the ten children files did not include updated immunization records available for review.

A child with a diagnosed food allergy did not have a allergy emergency plan available for review.

Based on the documentation gathered and interviews conducted with staff and administration it was determined a child was given yogurt on two different occasions unknowing the child had a diagnosed allergy to dairy.

A child with known special needs was able to get out of his classroom and into the playground without his caregiver's knowledge.

There were 2 children files that did not have a Health Statement.

One child file did not have a date of enrollement for the child.

There were 2 children files that did not have an address for the emergency contacts. One file was corrected at inspection

One employee is missing nine hours of required annual training.

Operation was utilizing the gym classroom for storage. In order to maintain that spaced licensed they would need clear it for it not to affect their capacity.

Five out of nine staff files were missing yearly training hours.

Three out of nine staff records were missing notarized Licensing Affidavit.

Based on video obtained during the investigation, a former caregiver treated a child who would not stay on her cot in a harsh manner.

Based upon video obtained, a caregiver was observed using inappropriate judgment and poor self-control when frustrated by a child.

Based on video footage, a caregiver was observed laying a child forcefully and covering the child's head with a blanket.

Fire extinguishers were last serviced on September 2019.

Gas inspection was last inspected on 08/08/2019.

A cot was placed in a fire exit while nap time.

Three children in the 3 to 5 year old class were being put to sleep during nap time with their heads covered. * Caregiver was asked to immediately uncover the child's head.
