One of six children's records reviewed did not have a current immunization record in their file.

Cleaning supplies and a fog solution were stored unsecured in cabinets in the PreK classroom, accessible to children. Note: This was corrected at inspection when the caregiver placed a child lock on the cabinet containing the cleaning supplies and removed the fog solution from the classroom.

A caregiver in the toddler classroom didn't wash hands after sanitizing the changing table. NOTE: This was corrected at inspection when hand washing was done.

Based on the interviews conducted with the teachers, parent and administration inadequate supervision was determined to have occurred when a child was found in the preschool classroom unsupervised.

In the toddler classroom a snack was provided on the bare table. This was corrected when napkins were provided for the children.

In the PreK classroom the caregiver stated there were 13 children in care when we verified there were 11.

One of the five staff files was missing medication training.

Lysol spray and cleaning wipes were observed in the PreK classroom accessible to children. NOTE: This was corrected at inspection when they were removed.

One of the six children files were missing immunization records at the time of the inspection.

Three of the six children files were missing the emergency addresses. One of the six children files was missing the healthcare/physician information.

One of the six staff members is missing 14 hours of annual training for 2022-2023

The director is missing 21 hours of required annual training for 2022-2023 which includes management training.

No documentation of the smoke detector, fire extinguisher or carbon monoxide was provided at the time of inspection. NOTE: This was corrected the inspection.

Two of the six staff files did not have 2021-2022 annual training for the following; Recognizing and preventing shaken baby syndrome and abusive head trauma. Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS) and Understanding early childhood brain development.

Toddler playground boarders was observed not to be secured as the screw was missing and protruding sticking up out of the black plastic boarders posing a tripping hazard. In addition to the bike ramp that was observed lifted posing a tripping hazard.

The following trainings were not available in the files for the six staff files: Emergency Preparedness Communicable diseases Food allergies Understanding building and premises and Handling, storing, and disposing of hazard material Four of the six staff did not have Abuse and Neglect training available.

One of the six children files is missing Authorization information. Two of the six children files were missing admission dates. (Corrected at inspection) Five of the six children did not have special care information. One child with no food allergy information. Three of the children files did not have either the address or phone number for emergency contacts.

In the PreK classroom cabinet under the sink a bottle of Bleach was found to be accessible to the children. No childproof locks was observed. NOTE: This was corrected as the bottle was removed to a cabinet with a lock and key.

Documentation of inspecting smoke detector, carbon monoxide, and fire extinguisher were not available for review.

Documentation of the building grounds checklist was available for review. NOTE: Corrected at inspection when the building grounds checklist was conducted.

The PreK classroom was observed eating on the floor while watching a movie. NOTE: This was corrected when the children were moved to the table to finish eating.

Children files were not available for review.

Both fire extinguishers in the building were last served on February 2019.

A current fire inspection was not available for review.

The PreK classroom was observed eating a snack while watching a movie. NOTE: This was corrected when the television was turned off.

The emergency preparedness plan did not specify procedures for; Communicating with authorities, Relocation diagram, Essential documents Continue care, and Reunification.
