During the investigation, it was determined that a child who swallowed a quarter was kept in care, even though they were vomiting repeatedly.

There were two medications that were expired.

There were two children's medication that were not labeled with the date brought into the operation.

There was no updated annual sanitation inspection. Last health/sanitation conducted 04/01/2025.

There were 2 out of 10 staff's files audited that were missing annual trainings for Communicable Diseases. There were 1 out of 10 staff's files audited that was missing the annual training for Building Safety.

Ten files were audited, 2 are missing current health stamens available for review during an inspection.

The playground was observed with a wasp nest, multiple ant piles, and garbage on all three playgrounds while children were present.

At time of incindet a caregiver did not have a current CPR/First Aid certification.

At time of inspection a PreK caregiver stated the class had ten children when their were eleven. This was corrected at inspection.

During a DFPS investigation, it was determined that operation staff failed to ensure that staff were informed of the number of children that should be present in a classroom when they came to assist during naptime, resulting in the operation failing to identify that a child was missing for over an hour and a half.

Operation staff failed to ensure adequate supervision of all children in their care when a child exited the building and was on the playground unattended for at least one and a half hours.

This standard was found deficient as part of a DFPS Investigation.

One of the ten children files didn't have an updated immunization record.

During naptime in the Infant C classroom one caregiver was supervising seven children with the youngest being 16 months and the oldest being 19 months old. NOTED: This was corrected when the 16-month-old was moved to the mobile infant classroom.

During inspection, two out of ten children files needed updated immunization records.

On 5/22/2023, there were two people present at the operation pending background checks and in care of children. One person was in the infant classroom and the other was in the 2 & 3 year old classroom.

Based on the information gathered and interviews conducted at the time of the incident the caregiver was wiping down shelves, cleaning and sweeping not directly involved with teaching, care and supervision of the children.

The Pre school and toddler playground were inspected to have less than 6 inches of mulch and rubber tile pieces.

The PreK 4 classroom children were observed out of control as children were running, hitting/throwing pillows at friends, not keeping hands to themselves, child flipping on the carpet, stuffing the sink with paper towels as the caregiver was getting ready to pass out snack. In addition to the caregiver passing out skittles for showing good behavior, and cleaning up the classroom.

The Consumer Product Safety was last certified on 1.1.21. NOTE: This was corrected when the website was reviewed a CPS form was posted.

An incident report was not used to document this prohibit item that was left in a child's bag and brought into the center to be unsafe.

Based upon information gathered as a result of this investigation and interviews conducted it was determined that the operation failed to immediately notify the parent on the situation that placed the child at risk.

Based upon information gathered and interviews conducted supervision was determined to have occurred after a three-year child was found in the kitchen unsupervised.

Based upon information gathered as a result of this investigation and interviews conducted it was determined that the operation failed to share an incident report for a parent signature of the incident.

Based upon information gathered as a result of this investigation and interviews conducted it was determined a Licensing Incident/Illness Report Form 7239 was not available for review.
