During a follow up, it was determined that no observations were conducted after initial inspection and forms were provided for that purpose. This is a recite of initial deficiency.

During a follow up inspection there was a screen time program playing during naptime. Children were awake during this time.

The fire inspection had not been scheduled for renewal and expired on 1/5/25.

While reviewing director qualifications, the director did not have verification of current transportation training.

During a follow up inspection four children were observed leaving their classroom unsupervised. A nine year old was accompanying two 5 year olds and a 4 year old as they returned to an empty classroom to retrieve a forgotten item.

During the course of a DFPS investigation, it was determined that staff failed to ensure that other staff were aware that children were being transitioned to their group. This failure to communicate regarding the transition of children from one group to another resulted in a child being left on the playground unattended for approximately 30 minutes, with nobody noting the child's absence until the parent arrived to pick up the child from care.

This standard was found deficient as part of a DFPS Investigation.

During the course of a DFPS investigation, it was determined that a caregiver failed to ensure that all children in their care were present when transitioning from outdoor play back indoors, resulting in a child being left alone unsupervised on the playground for 30 minutes.

Two of ten children's files reviewed, did not have an up to date immunization record. Two of ten children's files reviewed, did not have a signed physician's statement in their file.

The caregiver acknowledged that none of the toddler training cups were labeled. This was corrected at inspection when the assistant began putting labels on the cups.

Two of eleven employees did not have their CPR/First Aid certifications.

There were no provisions listed for moms who are breast feeding their child in the operational policies. This was corrected at inspection when a procedure was written to be added to the operational policies.

Child was taken out of class for an undetermined amount of time that is estimated to be over an hour as punishment

During investigation, it was determined that the operation withheld food from a child as punishment during a special event in the classroom due to alleged misbehavior.

A transportation van does not have a working electronic child safety alarm. The operation has added two additional vehicles for transportation use; one of which seats 8+ passengers and will need to have an alarm installed.

While inspecting both current transportation vehicles, it was observed that the emergency binder for transportation vehicles does not include medical emergency authorizations for children transported.

One of six employee files did not have a copy of how she meets education qualification for employment at operation Two of six employee files did not have employment affidavits in their personnel file.

Two of five children's files did not contain the signed admission form from their physician nor the health care provider statement from their physician. One of five files did not have a current immunization record on file.

On one of the child's immunizaitons reord reviewed three required immunizaitons were not documented.

Based on interviews with staff and children, a caregiver used inappropriate discipline by grabbing and yanking a child by the upper arm.
