Two of four children's records reviewed were missing admission information of: child's and parents address, admission date, parent telephone number, emergency contact, release information, emergency care authorization, and special care statement.

One of four children's records reviewed did not have a health statement on file.

Two classrooms did not have a smoke detector.

Operation did not submit proof of liability insurance annually by anniversary date of issuance.

At time of inspection, operations fire extinguisher was last serviced July 2024.

At time of inspection, three pet vaccinations were past due.

At time of inspection an infant younger than twelve months was observed inside the play yard with a blanket. Corrected at inspection.

At time of inspection an infnat was observed inside the play yard wearing a bib.

At time of inspection, operation could not provide childrens files for review.

During the investigation, it was made aware the operation has "at-door policy" that has parents drop off and pick up at the door.

Operation was out of ratio by one child, having one child under 18 months and six children over 18 months.

Based on documentation and interviews with provider and parents. The provider was out of ratio by have two infants under age of seventeen months.

Duriing the test of fire alrams it found no batteries were in the smoke detector. This was corrected at inspection when provider replaced the batteries and smoke detector was operating successfully.

One of the six files reviewed was missing a copy the immunizaiton records. This was corrected at inspection when provider was recieved a copy of the child's immunizaiton and placed it int the file.
