Additionally, on the basketball playground, a section of the fence leading to the other side, where the apartment complex is, was also not secure, permitting children to climb through to the other side.

There were five infants with one caregiver in the infant room. Three of those infants were under 12 months old. Note. A second caregiver was assign to the classroom.

There was a screw exposed on the play kitchen sink in the pre-k classroom. There was debris underneath the two different diaper changing mats. Note. The screw was removed from the play kitchen. The diaper changing stations were sanitize.

Two staff did not have current training on SID's, and Shaken Baby Syndrome.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required by field communication #340. Note. Operation complied after the required due date.

Last fire marshal inspection was conducted 09/08/2020.

Three fire extinguishers had expired service tags dated March 2020.
