Two caregivers provided an inaccurate count of the children in their care. One caregiver stated there were 19 children but there was 19. The other caregiver advised that there were 10 children but there was actually 11 children in the group.

There was a frayed seat belt in one of six buses used for transportation of children.

At time of inspection, a PreK caregiver stated they had seven children when they had eight.

Operation failed to notify Child Care Regulations Department within 48 hours of a confirmed Covid-19 incident at the center.

Loose-fill (rocks and mulch) were not evenly distributed on the playground leaving several spots with insufficient amount. This was corrected at inspection.

Evidence gathered during the investigation indicated that the caregiver was supervising two groups of children. The caregiver was standing in the doorway between the playground and the hallway while also completing an incident report, which diverted the caregiver's attention resulting in two children engaging in inappropriate behavior while using the restroom.

There was observed to be less then 6 inches of loose-fill surfacing material located under the swings and slide exits on the 3 and up playground. This was corrected at inspection when a caregiver racked some additional loose fill into those areas.

There was a child that was placed by the front door, that is used as an exit out of the classroom, for nap time. This was corrected at inspection when the director moved the child's cot and moved him to a different spot in the classroom.

A child was bitten by another child on the face while in care, resulting in significant swelling to the child?s eye and face. During the investigation, it was discovered the child who bit had history of biting prior to this incident. The caregiver saw this child walking toward the victim but did not intervene until after the biting had occurred; therefore, not ensuring the child?s safety. Additional bites were observed on the victim; however, there was not sufficient evidence to determine they were sustained while in care at the center.
