The diaper changing area was not sanitized after using it for an 11 month old infant.

The 3 dogs on the premises are missing vaccine records. This is scheduled for Febrary 2026.

A child did not wash her hands after having her diaper changed. This was corrected at the inspection.

The tv was on playing PBS Kids with an 11 month old in the room. The tv was left on for over an hour and did not follow the activity schedule. This was corrected at the inspection when it was turned off.

Primary caregiver [name redacted] is missing 25 hours of annual training.

Permit holder is not current on First Aid.

Permit holder does not have current vaccination records for the four pet dogs. They expired April/2023.

The home was found in unsanitary conditions. Bleach on the kitchen counter near food items. Garbage exposed and accessible to children. Significant amount of gnats were flying throughout rooms, around trash can and on garage door leading from kitchen. The under sink cabinet in the restroom had Drano accessible to children.

Operation has a total of eight children enrolled: Two children had no files available; Three of seven had no school information. Three of four files did not have a health care statement.

There was no written activity plan. Permit holder did not have an activity schedule to follow to keep children occupied.

The emergency preparedness plan was not available for review.

The operational policy did not specify the sleep requirements and restrictions for safe sleep for infants 12 months old or younger. -Pets requirements -Promoting indoor and outdoor activities -Duration of physical activities (indoor/outdoor) -Recommended clothing -Plan for weather permitting

One of the four children files was missing a immunization record.

Two of the four children files was missing a healthcare statement.

Two of the household members and permit holder did not have orientation available.

Children backpacks were observed on a recliner in the living room.

Permit holder did not have six hours of management training available for review. .

Dirty diapers were observed in the restroom trashcan in a unsanitary manner. NOTE: This was corrected when the trash was taken out.

Three of the four children files did not have address to the emergency contacts.

Three of the four children did not have release information.

An infant was observed being changed using a changing mat on the sofa. NOTE: A diaper changing pack and play was available to set up.

No current vaccination or healthcare statement were available for review.

Children were observed coming in from outside and not washing hands. NOTE: This was corrected when the children were assisted.

Children were observed sleeping on the couch, as mats weren't labeled with the children's name. NOTE: Mats were labeled with the children's name.

Carbon monoxide was not documented for the month of February and March. NOTE: This was corrected when it was tested and written on the Emergency practices.

The TV was observed on during the inspection while two toddlers were eating cereal. In addition to one of the children using a tablet. NOTE: This was corrected at inspection when tablet was placed in the child's backpack and TV was turned off.

The emergency preparedness plan did not specify what essential documents will be taken when evacuating. Or how they plan on continue the same care during an evacuation. And how you will reunify the children with someone not on the pick up list.

One of the four children files did not have school age information or permission on their enrollment form.
