The ceiling air conditioner vent in the school-age resting room is not properly secured and is held up by clear tape.

There was a caregiver in the infant classroom caring for children without a valid background check.

A caregiver left the classroom and shut the door behind her, leaving the children alone while she opened the main door for a parent and a Child Care Licensing inspector to enter the building. The children were unattended for about a minute because the parent had questions.

One caregiver did not know how many children she had in care, and another caregiver did not state the correct number of children she had in care. The caregiver had 13, but 14 was counted.

It was determined during the course of the investigation that the operation had insects such as cockroaches inside the building in almost every classroom.

During the course of the investigation, none of the staff members, including caregivers, the person in charge, and Center Directors, could identify who the caregiver was working with in the classroom.

The feeding schedules for the infants in care were not available for review. There were 2 missing feeding schedules.

During the inspection, the substitute director who was the Infant caregiver walked out of the classroom to answer the door leaving 3 infants unsupervised. They were all aged 11 months. This was corrected when she stepped back into the classroom.

During the inspection, a 9 month & 11 month old infant were in the 'Big Room' with the oldest child being 4 years old. This was corrected when the infants were placed back into the infant room.

During the inspection there were 5 infants in the Infant classroom ages 3 months, 9 months, 11 months, 11 months, and 11 months. The classroom is out of ratio due to the specified age group being 11 months.

Two of ten children's records reviewed did not have a health statement on file.

The last recorded fire drill was 11/21/2025. This was corrected at inspection when a fire drill was conducted.

Two infants who were not yet on table food did not have current feeding instructions.

A rubber full-size tire on the climbing structure in the toddler playground is shredded. The protective coating covering a steel step to the composite play structure separated, creating a sharp, jagged edge. The structure was immediately cordoned off with caution tape and the structure was put out of service.

A crack developed on the wall of the building large enough for a child to put their fingers in. This was corrected at inspection when the gap will filled with foam sealant.

Two of ten children's files audited did not have current shot records on file.

One of ten children's files audited did not have a health statement.

An employee was observed taking a puff of a vaporizer.

While working in a back room a child wandered in unsupervised. This was corrected when I walked the child back to his classroom.

A 17 month old infant was napping with the older children. This was corrected at inspection when the child was moved to a room with toddlers.

The loose fill under the climbing structures in both playgrounds is less that six inches.

The operation did not report to licensing an occurrence that rendered the facility unsafe for children.

The last fire inspection was conducted on 08/18/2020.

Two out of eight children's records do not have the doctor signed health statement.

Two pre-k children were observed leaving the classroom unattended to use the restroom in another classroom unsupervised.

Specified age group in a infant class was 16 months. The classroom was out of ratio when it had one caregiver with 6 infants.

A caregiver did not know the names of two of the children they were responsible for.

A caregiver stated she had 12 children in her care but physical count of the children showed 14 children in total. The caregiver was responsible for 14 children not 12.
