The cots were not all labeled with names/ numbers in the classrooms.

Director observed children outside of classroom headed out to the playground without a caregiver with them in the playground. The director did not address the issue while the incident was occurring. Note: The issue was addressed with the director.

There were three classrooms that had cleaning supplies accessible to children in care. Note: The cleaning products were removed out of the children's reach.

The facility's fire inspection was conducted on 06/21/2023.

The caregiver failed to supervise the children while they were in care, which resulted in a child getting injured.

Based on the information gathered during the investigation, there is sufficient evident to support allegations of caregiver using poor judgement when redirecting children behavior and or seating children in care.

During inspection caregiver training of discipline and guidance could not be observed.

The operation was aware of the ceiling leaking which resulted in mold in the Toddler ceiling requiring the children to be placed in a different classroom, however there was no report to Licensing. The operation was also aware that the toilets were at some point not functioning and there was no report to licensing.

The operation failed to notify the parents in writing of the alleged incidents.

At the time of initiation, the operation had not reported the incident involving the alleged cigarettes or marijuana found by a caregiver in a 15-month-old child's backpack. Nor had the operation reported that a 16-month-old child or caregiver had found or come in contact with a vape.

The ceiling in the Toddler classroom had been taped and sealed off due to leaking water and mold.

The children's products certification form was not available to review during inspection. Note: corrected at the inspection time.

During the inspection it was observed that the cots were not labeled.
