An accurate sign in and out log was not maintained when a caregiver signed a child out without verification the child left the operation.

A member of management did not use good judgement when they did not look into a staff concern promptly regarding the pick up of a missing child. This led to the child remaining missing for a much longer time frame.

DFPS determined a child was neglected while in care.

A child was not supervised appropriately. The child was left outside and found in a drain hole. The child was in the hole for approximately 1 hour and 13 minutes

drain cover was not secure over the splash pad water tank. This should have been noticed during the daily required outdoor checks to ensure the area is safe for child play. A barrier to the area or repair was not made before children had access to the area.

Based on the information gathered during the investigation, it has been determined the school failed to report an incident that required medical treatment to a child.

The CPSC/Recall products form 2885 was not available for review during the visit.

Four out of ten children's immunization records were not updated during the visit.

Three caregivers did not have current Pediatriac First Aid Training.

Three caregivers did not have current CPR training at the time of the inspection.

One child's emergency food allergy plan was not signed by a parent.

Six caregivers' files were missing 24 hours of annual trainings.

Four caregivers did not have signed employee affidavits in their personnel files.
