A group of children were engaging in outdoor activity with water not available to all children. Not having water available during the hot weather temperatures may cause heat exhausting or dehydration which places a child at risk.

Young children from 2 separate classes were observed engaging in outdoor activity with water not available. Not having water available during the hot weather temperatures may cause heat exhausting or dehydration which places a child at risk.

During the investigation, it was found that a single caregiver was left alone with 20 or more children with a specified age of three-years-old. The maximum number of three-year-old children a single caregiver can supervise is 15 children.

Despite being aware of a child's history of runaway behavior, a caregiver chose to enter the operation building without arranging additional supervision coverage for the child.

A child in care was found unsupervised outside of the operation.

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with the parent and staff, it was determined that a child did not have their diaper promptly changed.

This standard was evaluated as part of the investigation and was found to be deficient. In two restrooms where diapers were changed, there was not a changing table or a smooth, non-absorbent pad to change diapers.

The parent handbook was missing information about parent rights.

This finding was notified after the completion of a DFPS investigation. The operation was notified of a deficiency in Standard 746.1201(4) - Abuse and Neglect on 01/12/26 and failed to notify the parents of the deficiency by the allotted date, 01/17/26. The operation didn't notify the parents of the deficiency until 02/06/26.

Based on information provided from a DFPS investigation it was determined that a caregiver did not utilize positive statements when redirecting a child in care to move to another area in the room.

Based on information provided from a DFPS investigation it was determined that a caregiver grabbed a child by the arm and lifted them from the ground. The action by the caregiver caused substantial bruising to the child's arm.

Based on information provided from a DFPS investigation it was determined that a child in care was abused by a caregiver and the caregiver mentioned in the allegations has been found Reason to Believe.

This standard was tasked over for this investigation and found to be deficient. It was determined during this investigation that the child mentioned in the report was not given and/or offered water or a change of milk to drink through the school day. The child was allowed to only drink the same milk he arrived to care with throughout the day. the center director, has in place that the children's personal cups can only be filled with water at drop off time. Milk will be given throughout the day in disposal cups.
