An child in care did not have an updated infant feeding instructions form on file.

Two staff members did not have proof of training available for review handling, storing, and disposing of hazardous materials or preventing and responding to emergencies due to food or an allergic reaction;

Operation did not have a current consumer product safety form available for review. This was corrected at inspection when the director went to the consumer product safety web site then completed a new form.

A review of the Employees Who Left Employment in 2023 Report with a run date of 01/16/2023 indicates that the operation did not report the number of employees who left employment in 2023 by the required date of 1/15/2024.

While checking children's records, seven children did not have date of admission. This was corrected at inspection whe nthe director entered dates of admission in all seven records.

While reviewing children's records, two records did not contain documentation showing operational policies were provided.

While reviewing peronnel records, three caretakers did not have proof of orientation available for review.

While checking personnel records, three caretakers did not have a notarized Licensing Affidavit for Applicants for Employment form.

The operation did not have the most recent licensing inspection letter or form posted. This was corrected at inspection after the most recent licensing inspection form was emailed to the operation and was printed and posted.

During inspection, while evaluating records, two children files did not have immunization records.

During inspection, the operation did not have a completed children's products certification.

During inspection, while evaluating records, two children's files were missing the date of admission. This was corrected at inspection after director added the date of admission to the children's admission forms.
