The operation was one child over ratio in the infant classroom. There were 5 infants with a specified age group of 0-11 months in the classroom with one caregiver. A second caregiver came into the room with a sixth infant to come back into ratio.

There was a 12 month old child in a classroom with children ages 2-5. The child was moved into the infant classroom with a caregiver.

One caregiver was observed with 6 children and a specified age group of 0-11 months. This was corrected at inspection when a second caregiver was moved into the classroom.

Caregivers do not use highchairs for discipline. Redirection is primarily used for discipline and if needed, the time out area is a chair against the wall.

Five month old was obsrved sleeping in a crib with a pillow and teddy bear.

Caregiver left classroom with 5 children unattended and was out of auditorial and visual range.

One classroom was observed to be over ratio. The infant classroom had 1 caregiver to 6 infants.

Three month old infant was observed sleeping in a restrictive device with a blanket.

The operation had 1 caregivers in the building without an active background check. Caregivers was observed in the classroom alone with 6 infants.

The operation is not correcting deficiencies and has been recited for the same deficiencies.

Classroom was observed to have children 14month to 5 years old with one caregiver.

Infant classroom was observed with no teacher for 1-2 minutes.

Two classrooms were observed to be over ratio. The infant classroom had 1 caregiver to 6 infants. The toddler classroom had 1 caregiver with the specified age group of 2 years old with 14 children.

The operation had 2 caregivers in the building without an active background check. Caregivers were observed helping with children. Both caregivers left the building.

Lighting in the infant room too dark, unable to see infants in their cribs.
