An four month old infant was observed in the younger infant classroom to be asleep on their back and wearing a Maglic Merlin Sleep Sac.

Of the 10 staff files reviewed, all staff had CPR/First aid training that was online only and does not meet American Heart Association guidelines.

In a classroom with a specified age group of 15 months, there was one caregiver with 8 children. The ratio for this specified age group is one caregiver to every 5 children. This was corrected at inspection when a second caregiver came into the room.

Based on the information gathered during the investigation, it has been determined that the school failed to report to Child Care Regulations that a child had been involved in any on-routine situation that placed, or may have placed, the child at risk for injury or harm.

The staff in 2 out of 9 classrooms walked did not know how many children they had in care or their ages.

2 out of 10 children did not have an updated or actual vaccination record available for review.

Staff in 2 classrooms did not wash hands after assisting toileting for children.

The garbage can had no top covering the garbage. Located next the sink.

This standard was evaluated during the investigation and was found to be deficient. Caregivers left a child in a room alone and unsupervised for fifteen minutes after taking a group of children from one room and into another.

Documentation for fire drills has not been recorded since February 2021

Two out of ten childrens' records evaluated were both missing the following immunizations: Hep A (1) and DTap(1)

Two out of ten children's records evaluated did not contain healthcare provider statements.

Training records for nine out of ten personnel records were not available for review.

The annual fire inspection has not been completed for the year.
