The most recent licensing inspection was not posted. The last one available was from 01/29/2024. This was corrected at inspection when the most recent licensing inspection form was printed and posted.

The operational policies were lacking the parent's rights section.

The operation did not have a fire inspection completed in over 12 months. The last fire inspection was completed on 02/21/2024.

The playground had piles of leaves in the corner of the playground.

Four out of five staff records reviewed were lacking training in emergency preparedness plan, emergency food allergies, building and premise safety and handling and storing of hazardous materials.

The annual fire inspection was last conducted in August of 2022 and has not been re-done.

Staff lacked a current face to face CPR

Caregivers' records reviewed did not have the required 24 training hours for the last full year of employment.

One staff personnel record lacked a signed affidavit.

The Director had 20 hours of the required thirty hours of training for the last full year of employment.

The required Consumer Product Safety Commission form has not been completed within the last year. CI: This was corrected at inspection when a form was completed and posted.

Leftover macaroni from lunch was left uncovered. CI: This was corrected at inspection when the food was thrown away

One child was observed eating on the bare table. CI:This was corrected when plate was given to the teacher to use.

Caregivers showed a lack of judgment by using prohibited punishments and not following meal patterns.

During lunch, the fruit was observed to be withheld until the majority or all the food on the plate had been consumed by each child. A caregiver was heard during the inspection telling a child to eat their green beans and then she can have her fruit.

Based on interviews and observations children are given a cup of milk and are required to drink all of their milk and give the empty cup to a caregiver prior to being offered their lunch.

Based on interviews a child who screams while throwing a tantrum was placed in the restroom until the screaming had stopped.

One of six children's records evaluated did not have a health statement on file.

The last fire inspection available for review was conducted on 01/07/2021.
