PISD Timmerman Elementary Extended Day Program

A caregiver failed to supervise a child when the child was left in alone a classroom.

One child was missing medication listed on their food allergy plan.

Information obtained throughout the investigation shows that a caregiver has had children sit quietly at a table for 10-20 minutes.

Information obtained throughout the investigation shows that a caregiver made a child sit facing the wall as a form of punishment. When the child turned their head around, the caregiver used their hand to turn the child's head back towards the wall.

The operation did not report a change in director.

A director was not listed as a controlling person for the operation.

The operation did not have the letter or form from the most recent Licensing inspection or investigation posted.

Two children with diagnosed food allergies did not have a food allergy emergency plan.

The list of child's allergies observed on the bulletin board was not up to date. This was corrected with the director updated the food allergy list.

Three bottles of hand sanitizer were observed accessible to children in the main classroom.

The last monthly maintenance checklist was completed in Nov2023

Children did not wash hands or used hand sanizter prior to eating.

One caregiver obtained CPR certifications from an online course, NationalCPRFoundation.

Observed a bottle of cleaning soultion sitting on the desk next to the door of the main classroom.

The list of child's allergies observed on the bullentin board was not up to date.

Last fire drill was completed in November of 2023.

During the follow up inspection, observed the caregiver sitting in a chair, near the entrance of the school, out of proximity of the children who were playing in the back field of the outdoor space.

The caregiver was unable to tell how many children were present in the classroom upon request. This was corrected when the caregiver reviewed her roster and counted the amount of children in the class.

Based on the information obtained, there is enough evidence to determine that the operation did not appropriately intervene prior to a child sustaining an injury caused by another child in care.

It was determined that the operation was not adequately supervising the children to intervene when a verbal altercation disrupted between children that led to a physical altercation during outdoor time.

Upon arrival, one caregiver was supervising 35 students, ages 4 to 10 years old. This was corrected when another staff member arrived to the operation.
