On the toddler playground the posts from the boarder around the loose fill is are sticking up causing a tripping hazards. On the Pre-K playground the stairs leading up the climbing structure are rusted and the protective coating is peeling off.

The emergency drills were not documented with a start time only a finish time.

During the inspection on 3/21/2024, the operation did not have a current fire inspection on file. I followed up to request updated documentation and as of today, a new fire inspection has not been conducted.

The operation s fire inspection is not current as the last inspection was conducted on 1/17/2024.

A caregiver who did not have a background check was present in the Toddler1 classroom and had access to children in care.

In the Infant 2 and Nursery classrooms, diaper changing supplies were in unlocked cabinets that were accessible to children.

In the Toddler 3 and Toddler 2 classrooms, the children's belongings were hanging on hooks that were not labeled.

One out of ten employees are missing training in the following: -Handling, storing, and disposing of hazardous materials. -Administering medication. -Preventing and responding to emergencies due to food or an allergic reaction.

The director is missing 6 hours of annual training between the time frame of 7/1/2023 to 7/1/2024.

The operation's annual sanitation inspection expired on 10/6/2023.

In the infant 2 classroom, a fan was not mounted and was accessible to children. Note: This was corrected when the fan was removed from the classroom.

In the infant 2 classroom, multiple bottles of diaper rash cream were in an unlocked cabinet that was accessible to children. Note: Corrected when the items were moved to a location that was inaccessible to the children.

Two out of ten children's files evaluated were missing current immunization records on file for review.

The infant 1 classroom was not within ratio as one caregiver was with 9 children, ages 12 to 19 months.

The operation's emergency preparedness plan does not state how children under 24 months will be evacuated and relocated during an emergency.

Out of 10 files reviewed 7 of them did not have the following required training topics were missing: -Emergency Preparedness -Preventing and controlling the spread of communicable diseases, including immunizations -Administering medication, -Understanding building and physical premises safety

In the nursery there were no feeding instructions available for the 6 infants in care.

In one classroom there were 3 outlets that did not have covers on them. Note: This was corrected when the director replaced the outlet covers.

The fence around the playground needs a wood piece replaced.
