Young children were left unsupervised inside a classroom when the caregivers went into the restroom. A young child was observed climbing on top of a table inside the classroom.

Children were observed (via video footage) engaged in screen time that was not integrated into a planned educational lesson plan or activity.

There were multiple holes on the changing pad.

The crib sheet was not the correct size, causing the sheet to be scrunched and loose fitting.

There were three sippy cups found next to children while sleeping. Two sippy cups were next to cots and one sippy cup was in a crib.

The Explorers Changing Pad had visible debris. The carpets in the classrooms were stained and needed cleaning.

One child did not have an Allergy Emergency Plan in their record.

Two of the ten children's records reviewed did not have updated immunizations in their records.

Fire extiguisher checks were missing for the months of January and February.

Two outlet covers were missing. One in the Camper Room and one in the Inventor Room.

A caregiver did not use good judgment when falling asleep while holding a baby in arms.

A child was asleep on a cot that was blocking the front entrance.

It has been determined that a child was left unattended outside.

Based on preponderance of evidence a child had access to an e-cigarettes while in care.

The is sufficient amount of evidence to support allegation of caregiver not supervising children while outside on the playground. Also another caregiver fell asleep while supervising children ages 2 to 3 during nap time.

Based on information obtained, there was sufficient evidence to support a caregiver physically disciplined multiple children. Note: The caregiver no longer works at the operation.

Based on information obtained, there was sufficient evidence to support a caregiver did not use good judgment.

During the follow up inspection a caregiver was observed in the classroom caring for 10 children without a cleared background check.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Based on the evidence gather it is determined that the caregiver in charged of the school age classroom was mopping while the children were in care.

The operation did not have any documentation of fire drills.

The sheet in the infant crib was loose and did not properly fit the crib. This was corrected when the sheet was replaced.

Based on the information obtained, there is sufficient information to show that supervision was not properly maintained due to a child being left alone outside.
