When arrived at inspection one caregiver was outside pushing 5 infants, ages 8 months to 12 months (SPA: 12 months) in a Bye Bye buggy leaving a second infant caregiver inside the classroom with 5 infants, ages 4 months to 9 months. These numbers exceed the ratios allowed in a child care center. This was corrected at inspection as the caregiver took the children back to the infant class that had an additional caregiver to helps supervise the children.

A sampling of children records were evaluated. Two children's immunization record was observed in complete. Discussed with the director the name of the children and their missing immunizations.

This standard was reviewed as part of an investigation and found deficient. It was determined that staff did not document the amount of food consumed by an infant on the infants written daily reports.

Five infants written infant feeding instructions had not been reviewed or updated by the parent or health care professional in the past 30 days.

Black tarp and clear plastic were protruding from the ground on the playground posing as a tripping hazard. This was corrected at inspection as the tarp and plastic were removed. Two wooden boards were missing from the fence and need replaced.

One caregiver who was in charge of a group of children, at inspection, did not have documentation of current first aid training.

One of the directors lacked documentation of completion of 18 clock hours training to complete last years annual training.

The operational policies were evaluated and lacked the following written policy: Promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter (relating to Developmental Activities and Activity Plan); your policies must include: (A) The benefits of physical activity and outdoor play; B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (D) Each setting in which your physical activity program will take place; (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.

This standard was reviewed on 9/22/2022 as a follow-up to an investigation and found deficient. One caregiver was observed supervising five children, ages 13 months, 10 months, 7 months, 7 months, and 7 months. These numbers exceed the numbers allowed in a Child Care Center. This was discussed with the person in charge. This was corrected at inspection as a second caregiver was sent into the room to assist supervise the children.

This standard was reviewed as part of an investigation and found deficient. A caregiver whose presence at the operation is on a provisional basis and must never be left in charge of a group of children was observed working in a classroom alone with and in charge of a group of children. This was corrected at inspection as another staff member went into the classroom with the staff member. In addition, per interviews conducted it was determined this individual has worked alone in a classroom with children in the past.

One caregiver was supervising 8 children, ages 4 months, 14 months, 14 months, 14 months, 15 months, 15 months, 16 months, and 18 months. These numbers exceed the ratios allowed in a Licensed Child Care Center. This was discussed with the director. This was corrected at inspection as a second staff member came to the classroom to help supervise the children.

This standard was reviewed on 12/8/2021 as a follow-up to a previous inspection and found deficient. Two other boards on the fence have come lose and need secured. Two nails were protrudint from one wood board in the fence. There is a concrete block on the playground. A space heater was observed being used without a safegaurd to prevent access to the children.

One caregiver was supervising 14 children, ages 4 months to 9 years. You may combine infants with older children as long as the oldest child in the group is not more than 18 months older than the youngest child.

A wood board on the fence around the playground has broke off and needs repaired. Two other boards on the fence have come lose and need secured. Nails were protruding from a wooden structure on the playground. A small hole was observed in the wall in one room. Part of one air vent cover was not secured to the wall. A second air vent cover did not completely cover the air vent, exposing a small portion of the vent.
