The operation did not have a most recent sanitation inspection report available for review.

A recent Fire Inspection was not available for review.

A Fire Drill for January 2026 has not been done.

The children's products certification form is not up to date. The last time it was certified was January 8th 2024.

Upon reviewing children's records, all of the information that is required for parent to sign off on regarding operational policies, was not provided on the admission form.

An employee was found to use inappropriate discipline on a child in care.

This standard was found to be deficient as part of a DFPS investigation.

At my arrival, I was informed that director was not present. Staff did not know who was in charge when director is not present.

There are three children who have a diagnosed food allergy. One child did not have his signed by parent; another child did not have a completed food allergy plan. None of the plans were posted in the child's designated rooms.

Two caregivers got into a verbal altercation in the presence of children.

An employee had her own 3 year old child in a room with infants younger that 17 months of age.

I observed the infant room out of compliance. There was one caregiver with 6 children ages 4 months to 17 months. However there is also a developmentally delayed child in the classroom who is 27 months.

Upon reviewing children's records, one child did not have the special care statement addressed.

Two staff who do not meet the education requirement were being used in ratio in a classroom and to supervise children.

The Children's Products Certification form was last certified in September of 2020.

During my walk through, I observed the barrier in the outdoor area that had two bolts sticking out, creating a safety hazard.
