During the walk through it was observed that one classroom with toddlers the nap mats were not spaced to allow the caregiver to walk without stepping over the children. There were children also under equipment and on equipment with their mats due to space. Note: Pictures were taken. This was corrected at inspection when the nap mats were rearranged.

One out of ten child records reviewed is missing the food allergy plan for a diagnsoed food allergy.

The emergency preparedness is missing information about how to evacuate children 24 months and younger or with limited mobility.

Two out of ten staff files reviewed did not have the education documentation.

A composite play structure was observed with swing and slide attached in one structure.

During the inspection, one of the two caregivers supervising 6 infant children left the classroom to open the front door. The median age of the group of infants was 13 months. The required child/caregiver ratio for 12-17 months is one caregiver for every 5 children.

I did not observe documentation of an immunization record in 2 out of 8 children's records evaluated.

I did not observe documentation of the center's safety drills that they have conducted.

I did not observe documentation of current certification in SIDS, Shaken Baby Syndrome or Abusive Head Trauma for the caregivers who work in the infant classroom.
