This standard was reviewed on 5/1/2025 as a follow-up to a previous inspection and found deficient. Two children's records lacked documentation that their parents received a copy of the operational policies. One child's record lacked health statements. Two children's immunization records were observed incomplete.

This standard was reviewed on 4/11/2025 and found deficient.One piece of equipment on the playground was observed with a portion of the equipment being removed and now does not meet manufacturers instructions and there is nothing to prevent the children from falling off the equipment where the pieces have been removed.

The primary caregivers first aid training expired 11/2024.

The last documented fire drill was observed 9/2024. Fire drills must be conducted monthly. The last documented lock down drill/severe weather drill was observed 1/2024. The last documented carbon monoxide and smoke detector checks were observed documented 5/2023.

The primary caregivers CPR training expired 11/2024.

A sampling of children records were evaluated. Two children's records lacked documentation that their parents received a copy of the operational policies. Three children records lacked health statements. One child's record lacked an immunization record. Two children's immunization records were observed incomplete.

The operation did not have a statement of health for the dog at the home on file for review by Child Care Regulation.

An individual was present at inspection without a background check.

One piece of equipment on the playground was observed broken and needs to be repaired or removed.

The household member lacked documentation of completion of orientation.

The operation did not have a written activity plan.

The primary caregiver lacked 19 hours of training to complete last years annual training.

A sampling of children records were evaluated. Two children's immunization records were observed incomplete.Discussed the names of the children and their missing immunizations with the primary caregiver.

The last documented annual certification on file that the operation reviewed the CPSC recall list and certified there were no recalled items in the home was observed 6/2021. You must certify annually in writing that you have reviewed each of the recall notices issued by the CPSC and that there are no unsafe products in the home.

The operational policies were evaluated and lacked the following written procedures: 1) Safe sleep policy; 2) Promotion of indoor and outdoor physical activity (A-C); 3) procedures for parents to visit the home any time during hours of operation to observe their child, program activities, the home, the premises, and equipment without having to secure prior approval; and 4) the HHSC website (www.hhs.texas.gov).

This standard was reviewed on 3/17/2022 as a follow-up to a previous inspection and found deficient. The metal frame of the swing set that was taken down is rusted and on the playground accessible to children. It needs to be removed from the playground. A three person wooden swing with stand was observed on the playground with wooden boards missing from the seat and the back of the swing posing an entrapment risk to children. This was corrected at inspection as the swing was taken off the frame and removed from the playground.

A metal cover on a vent attached to the outside of the home is not secured and is sticking out. One of the legs on the frame of the swing set is bent making the swing set unstable. This swing set needs to be removed from the playground. The roof tops of two play houses have large areas of missing plasttic as well as cracked plastic. One piece of equipment with a slide has a hole in the plastic at the end of the slide as well as another hole toward the top of the piece of equipment. The a/c unit on the playground does not have a safeguard around the unit to prevent the children from having access. The gates to the fence around the playground are not secured to prevent the children from exiting the playground.
