Based on the information obtained, there is sufficient evidence to suggest that a caregiver did not demonstrate competency, good judgment, and self-control in the presence of children.

In three different classrooms children were observed to not have their hand washed after toileting and after a diaper change.

2 out 10 staff record's evaluated did not have notarized affidavit in the file. Corrected at inspection : the operation was able to notarized the affidavits for the two staff files with no notarized affidavit in the file.

Bathroom faucet in the classroom was loose and not adhered to the sink.

Two caregivers in two different classroom stated that they did not know the ages of the children.

Caregiver was observed to not be in the classroom while children were napping.

5 out of 10 staff records evaluated did not have photo ID in the files. Corrected at Inspection : the operation was able to make copies of the five staff records with no photo ID and place the photo ID in the staff's files.

10 out of 10 children's records evaluated did not have the Parent's Rights form in the file.

8 out 10 staff files evaluated did not have the preemployment affidavit in the file. Corrected at Inspection: the operation was able to have the 8 staff complete the preemployment affidavit during the inspection.

1 out 2 vehicles did not have the child safety alarm on the vehicle.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

One child 18 month old in the Infant B classroom was observed sitting on a high chair without the safety strap fastened during lunch time. NOTE: This was corrected, the caregiver fastened the safety strap for the children during the inspection.

Child Care Regulartory was not notified by the operation regarding the change of the director since September 2021. NOTE: Documentation for the director in charge were received during the inspection.

The transportation training for three employees who transport children were not available for review.

It was observed that the diaper chaning surface in the Toddlers classroom was not sanitized after each use.

The children in Infant A classroom was observed not having their hands cleaned after diaper changing.

A five month old infant from the Infant A classroom was observed sleeping on the electric bouncer during the inspection walkthrough. NOTE: This was corrected, the caregiver removed the infant from the bouncer and place the child in the crib.

Diaper changing procedure was observed, and the caregiver in the Infant A classroom was observed not washing their hands after changing diaper for an infant.
