Three out of ten children's files reviewed are missing immunization records.

Three out of ten staff files reviewed are missing form 2912 (preemployment affidavit) and two are missing form 2985 (notarized affidavit),

Diaper changing mats in two classroom were torn. Note: This was corrected at inspection when provider replaced the mats.

Eight out of ten children's files reviewed are missing immunization records.

Two out of ten personnel files reviewed were missing records of training hours.

A caregiver was observed caring for 6 infants between the ages of 0-11 months. According to the minimum standards ratio chart, the maximum number of children a caregiver may care for is 4.

The sign in and out log has not been maintained. The initials of the parent/caregiver have not been done for the week. One classroom did not have the 11 children signed in; however, it was corrected during the inspection.

All the training certificates reviewed did not have an assessment tool to determine the information acquired from the training topics.

The vents in four classrooms were observed with dust and debris.

During nap time there two classrooms in which the mats were arranged right next to each other with no walk and work space for caregivers and children.

There were no current infant feeding instructions for 8 infants.

There was a bucket with water and a mop outside the infant room door in the outdoor play area. Note: The bucket and mop were removed.

A school aged child was observed in a toddler room with a group of toddlers and infants between the ages of 12-17 months. The oldest child in the group was 5 years old and the youngest child was 16 months old. The oldest child in the group was more than 18 months older than the youngest child.

A caregiver was observed caring for 6 infants between the ages of 0-11 months. According the minimum standards ratio chart, the maximum number of children a caregiver may care for is 4. Note: A second caregiver stepped in the infant room during the inspection.

An electric fragrance wax warmer was observed in a toddler restroom. The wax warmer was connected on top of the hand washing sink accessible to children. A total of five electrical outlets without a safety cover were observed in different rooms. Seven cans of disinfecting wipes were observed under a children's hand washing sink in a toddler room. There was no latch on the doors preventing the cabinet from opening. Note: This was CORRECTED during the inspection as the wax warmer was removed from the children's restroom. Safety covers were placed on the electrical outlets. The disinfecting wipes were removed and stored out of children's reach.

Soiled diapers were observed in a trash can without a cover in a toddler restroom. Note: This was CORRECTED during the inspection as the trash can was replaced with another that had a cover.

A list of each child s food allergies was not posted in the kitchen. Note: This was CORRECTED during the inspection as a list was obtained and posted in the kitchen area.

Children were served their afternoon snack (cookies/crackers) on a bare table in a toddler room.

There was no food allergy emergency plan available for review for two children with a known food allergy.

The feeding instructions available for review for three infants not yet ready for table food were not current.

2 of 10 children's files reviewed were missing the children's immunization record and 1 child's immunization record was not current. 1 of 10 children's files reviewed was missing the child's health care information.

2 of 10 personnel files reviewed were missing documentation showing how the employee meets the minimum age and education qualifications.
