Salinas Elementary Head Start Campus

Child/caregiver ratios were not met in a pre-k classroom when a caregiver stepped out of the room to conduct diaper changes leaving a caregiver to supervise a group of 17 children. The specified age of the children in the group was 3 years of age. According to the ratio chart the maximum number of children one caregiver may supervise is 15.

Operation has not provided parents a written copy of the parent's rights.

Parent Handbook was reviewed and the following information is missing: (13) How and when parents will be notified of policy changes. (22) Procedures for parents to review a copy of center's most recent CCR inspection report and how parents may access the minimum standards online.

Based on the information obtained during the course of the investigation, operation did not have prior background check eligibility notice for staff member / contractor that assisted and/or provided care to child during the incident.

Based on the information obtained during the course of the investigation, operation failed to timely notify Childcare Regulation of an incident that took place at the operation that required medical attention. Note: This was corrected during investigation when operation provided the information required to complete a self-report.

One out of ten staff training records reviewed was not available for review at the time of inspection. A second file was missing training certificate for training on communicable diseases. A third file is missing first aid / cpr training certificate.

One out of ten staff training records reviewed is missing pediatric first aid / CPR training.

Daily schedule does not meet the 60min outdoor play and 90 min physical activity requirements.

The most recent Licensing form certifying that operation reviewed each of the bulletins and notices issued by the United States Consumer Product Safety Commission was not available for review at the time of inspection.

Three staff members have been present at operation and routinely caring for children without background check eligibility notice. Note: Background checks were requested during inspection and staff members were advised to not return to operation until background check eligibility is received.

Children were participating in physical activity at the operation's gym, the gym has two fans and one of them was on and accessible to children. There are two wooden storage boxes without ventilation holes with an outdoor latch that could close and trap a child inside. The boxes are in the gym and accessible to children.

Children are participating in less than 30 minutes/day of outdoor play.

Ten staff training records were reviewed. Certificates for management training for one staff member and certificates for a second staff member were not available for review at the time of inspection.

It was determined that Salinas Elementary Head Start Campus is operating without a permit.
