A seat in the transportation vehicle is missing plastic cover exposing metal components. The vehicle's outlet plug was out of place and exposing some of the electric cables. Note: This was corrected at inspection when provider placed and taped outlet plug back into place and missing seat part was obtained and installed.

The operational policies reviewed were missing the following information: Preventing and responding to abuse and neglect of children, including: (A) Required annual training for employees; (B) Methods for increasing employee and parent awareness of issues regarding child abuse and neglect, including warning signs that a child may be a victim of abuse or neglect and factors indicating a child is at risk for abuse or neglect; (C) Methods for increasing employee and parent awareness of prevention techniques for child abuse and neglect; (D) Strategies for coordination between the center and appropriate community organizations; and (E) Actions that the parent of a child who is a victim of abuse or neglect should take to obtain assistance and intervention, including procedures for reporting child abuse or neglect;

A caregiver's record reviewed was missing the following information: (1) Documentation showing the dates of the first and last day on the job; (2) A statement signed and dated by the employee showing he has received a copyof the child-care center's: (A)Operational policies; and (B)Personnel policies; and (3)A statement signed and dated by the employee verifying the date the employee attended training during orientation that includes an overview of your policy on the prevention, recognition, and reporting of child maltreatment outlined in rule ?746.1303.

A bottle of hand sanitizer was observed on top of a sink accessible to children in a Pre-school classroom. Note: This was CORRECTED during the inspection when the caregiver stored the hand sanitizer out of children's reach.

An emergency evacuation and relocation diagram was not posted in the Pre-school room. Note: This was CORRECTED during the inspection as the director posted a copy of the emergency evacuation and relocation diagram near an exit of the room.
