Operational policies were reviewed and the following information is missing: (8)(f) Outdoor play extreme weather criteria; (20) Parent's rights; and (30) Procedures for supporting inclusive services to children with special care needs.

10 staff files were reviewed. Two file are missing high school diploma and two other files are missing foreign education translation. One file is missing notarized affidavit (form 2985).

There was no a food allergy emergency plan available for review for a child with a known food allergy.

There was no safety documentation available for review for emergency drills, fire extinguishers, smoke detectors, and carbon monoxide detector.

The director was missing annual training in the following topics: . (1)Recognizing and preventing shaken baby syndrome and abusive head trauma; (2)Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3)Understanding early childhood brain development. (1)Emergency preparedness; (2)Preventing and controlling the spread of communicable diseases, including immunizations; (3)Administering medication, if applicable, including compliance with ?746.3803 of this chapter (relating to What authorization must I obtain before administering a medication to a child in my care?); (4)Preventing and responding to emergencies due to food or an allergic reaction; (5)Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6)Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

Three caregivier's records reviewed were missing a notarized Licensing Affidavit for Applicants for Employment form.

Caregivers personal purses were observed inside a sink cabinet in a pre-k room. In a second pre-k room two cans of disinfecting wipes and three cans of lysol disinfecting spray were stored inside a sink cabinet. The safety latches to the cabinet doors were broken and the purses and cleaning supplies were accessible to children. Note: This was CORRECTED during the inspection as the purses and cleaning supplies were removed and stored in an are out of childrens reach.

Two caregivers with education received outside of the United States did not have the education documentation translated into English.

A child in the infant room did have their hands washed after having a diaper change. Note: This was addressed with the caregiver and the child was taken to wash their hands.

Drinking water was not provided to a group of children after playing in the outdoor playground.

Notification was not received when there was a change in space. A toddler room and infant room were switched.

The emergency practices form was not maintained. The safety tests documentation for fire extinguishers, smoke detectors, and Carbon Monoxide detector were on August 2022.

Soiled diapers were observed in waste baskets without a lid in two children's restrooms.

A group of children of varying ages including children under 2 years of age were sitting on the floor watching a cartoon show on an ipad in the infant room.

1 of 8 children's records reviewed were missing the child's emergency contact address. 1 of 8 children's records reviewed was missing the authorization signature to obtain emergency medical care and to transport the child for emergency medical treatment. 1 of 8 children's file reviewed did not include the child's immunization record.

Caregivers did not ensure that three children wash their hands after a diaper change in the infant room. note: This was addressed during the inspection and the children's hands were washed.

A caregiver used the same pair of disposable gloves and did not discard the gloves in between diaper changes in the infant room.

The emergency exit gate leading into the outdoor playground was locked. Caregivers supervising a group of children did not have the key to the lock while in the outdoor playground. Note: This was addressed with the director and the key to the locks was provided to the caregivers.

A caregiver's education documentation was not translated into English.

The operational policies reviewewed were missing the following information: The Promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter (relating to Developmental Activities and Activity Plan); policies must include: (A) The benefits of physical activity and outdoor play; (B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (D) Each setting in which your physical activity program will take place; (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.
