The diaper changing mat in the toddler room was observed with tears. This created the diaper changing mat to become absorbent and difficult to sanitize. Note: This was CORRECTED at inspection as the diaper changing mat was replaced.

The most recent Child Care Regulation inspection form was not posted. The date of the CCR inspection form posted was for an inspection conducted on 12/17/2025.

Bottles were being washed in the hand washing sink in the infant room. A bottle brush and dish soap were observed on top of the sink.

The emergency practices form 7263 was not being maintained. There was no documentation available for the monthly fire extinguishers checks, smoke detectors checks; and Carbon Monoxide detector checks. The date of the last monthly check on the form was 1/20/2026.

There were no written feeding instructions available for review for three infants who are not yet ready for table food.

Three cribs in the infant room were observed without a fitted sheet.

The director and a caregiver did not have current training in transportation safety. The date of completion on the training certificate available for review was March 2025.

The written activity plans posted in the Infant, Toddler, and After School rooms were not current. The date on the activity plan in the infant room was for the week of 3/23-27/2026. The date on the activity plan in the toddler room was for the week of 3/16-20/2026. The date on the activity plan in the After School room was for the week of 2/2-6/2026.

A caregiver caring for a group of children in the toddler room did not have at least eight hours of the required pre-service training before being counted in the child/caregiver ratio.

An infant's diaper rash cream was observed stored in a basket under the diaper changing table within children's reach in the infant room. Note: The diaper rash cream was removed from under the changing table and stored in an area out of children's reach.

A careigver caing for a group of chidlren in the toddler room did not have training in Pediatric First Aid.

1 of 4 personnel records reviewed was missing the caregiver's education documentation. 2 of 2 personnnel records reviewed were missing a notarized Affidavit form 2985, and a Pre-Employment Affidavit form 2912.

The operation was not practicing a monthly fire drill. According to staff the last time a fire drill was practiced was on January 2026. The last fire drill practice date documented on the emergency practices form 7263 was 1/19/2026.

A television was placed on top of a children's shelf in a toddler room during the naptime. The television was not anchored and within children's reach. Note: The television was removed from the room and stored in the office.

Two infants were left unsupervised for an undetermined amount of time in the infant room when the caregiver supervising the group was observed walking oit of the pre-k room holding an infant.

An infant over 12 months of age was observed asleep on the carpet without a mat/cot in an infant room. Two school aged children were observed resting on the floor without a mat/cot during the naptime. Note: This was addressed during the inspection and a cot was provided for the children.

The electronic smoke alarm system was not currently tested. The documentation available for review indicated the date of the last inspection was on 2/12/2025 and an inspection is due annually.

Peeling, bubbled paint was observed around a ceiling vent in the infant room.

There was no diaper changing table and an adult-sized rocker or chair in infant room #2.

Three electrical outlets did not have safety covers in the toddler room. This posed a safety hazard as the electrical outlets were within children's reach. Note: This was CORRECTED during the inspection as safety covers were placed on the electrical outlets.

Broken blinds were observed in the toddler room. This posed a safety hazard as the broken blind slats were withing children's reach. Dried plants were observed in the outdoor playground within children's reach.

There were no interest centers or age-appropriate seating, and tables for children in the pre-k room.

The First-Aid kit in the transportation vehicle was incomplete. The following items were missng: Thermometer, and Waterproof, disposable gloves.
