It has been more than 12 months since the fire extinguishers have been serviced. They were last serviced May 2021.

The new director has not been added as a controlling person.

There is no current health inspection on file. The last was was one on 8/2021.

A fire drill has not been completed since January 2022.

There were several items that the children have access to when out on the playground. There is a missing gate to the enclosure for the AC unit. The storage building outside is not locked and has items such as paint cans stored inside. There is a large gap of about 9 inches under the storage that is not covered which can allow children to crawl under the bottom. There is a broken pallet that the children have access to. The permit holder did not notify licensing that a new director had been hired.
