In the pre-k playground it was observed that the loose fill was not at the appropriate level of 6 inches in all use zone areas and leaving dirt exposed.

One of the ten staff files reviewed was missing current training in pediatric cpr.

The operation has not conducted a lockdown drill in the past year.

The transportation binder that is carried on the school vehicle was missing the authorization forms for children to receive medical treatment. This was corrected at inspection when the operation added the authorization forms for the four children being transported to the binder.

The transportation vehicle did not have a fire extinguisher. This was corrected at inspection when the operation mounted a fire extinguisher in the vehicle.

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff and children, it was determined a caregiver grabbed a child above their wrist and pulled the child by their arm across a room to a restroom

When a parent expressed a concern to the director about their child being inappropriately disciplined in care, the director did not report the incident that may have placed the child at risk for injury or harm to Child Care Regulation.

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff and children, it was determined that a caregiver took a child into a restroom with an obstructed view to discipline a child. It was determined the caregiver put their hands on the child's shoulders and pushed the child against the restroom wall which caused the child to hit the back of their head on the wall.

In the younger infant classrooom, the feeding instruction for the children present were not signed by a parent or had not been updated with the last 30 days.

In the older infant room, sheets in two cribs had food particulate on them despite none of the children in care having used the cribs today.

In the mixed age classroom (ages 3, 4 and 5) the electrical outlets were uncovered and accessible to children.

In the older infants classroom, a child's bottle was not labelled.

There was a hole in the wall observed in two year old classroom.

Broken toys and equipment were located on the playgrounds. These presented pinching hazards and due to being broken, cannot be used according to manufacturer's instructions. Several play structures had missing bolts in toys attached to play structures. Several uncovered holes were located in the ground that presented a tripping hazard. Animal feces were located on the splashpad accessible to children. The ground under the edge of the spash pad had eroded and presents rough surfaces that may scratch children and has a wire accessible to children.

In the older infant room, the fridge that stores children's food and drinks had uncovered food and was not clean.

In an infant room, the bottles were not labelled with the infants' names.

The operation did not have a fire inspection on file for the current ownership.

The operational policies were missing the criteria that the operation will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play.

On the toddler playground, there were protruding bolts coming out of the edging materials surrounding the playground. On the PreK playground, on the ground, there were electrical wires in a round box that did not have a cover on them.

On the school age playground, there were 4 slides that exceeded 5 feet in height. The depth of the mulch under the slides was 5 to 7 inches in depth.

The operational policies were missing the Safe sleep policy for infants birth to 12 months old.

The operation did not have a current gas leak inspection on file or proof that a gas leak test is not required.

The operation did not have a health inspection on file or proof that a health inspection is not required.
