The standard was evaluated for a standard investigation and was found not in compliance. Based on interviews with the staff and the parent of a young child, a caregiver grabbed a young child's arm to stop them from running away which resulted in the child falling and hitting their mouth on the pole of a play structure.

The operation has not conducted a current CO detector. The director corrected the deficiency during the inspection by testing the CO detector test.

The operation does not have a current fire inspection conducted by a fire marshal. The last fire inspection documented 8/24/2023.

The operation did not have the current Children Product Certification form to validate that the CPSC recall list have been view and all of the materials in the operation that are used by children are safe. The Children Safety Product Certification form was last documented 8/26/2023.

Two caregivers did not have documentation on file acknowledging that an orientation is complete.

The operation does not have a current health inspection. The last annual health inspection is documented 2/28/2024.

Based on interviews with the staff and the parent witness, the operation was found out of the compliance with the standard evaluated. The caregiver was observed hitting a young child on the wrist.

Based on interviews with a caregiver and a parent witness, a caregiver was observed hitting a young child to redirect the young child's behavior.

A child did not have a health statement on file.

Three children did not have current immunizations on file.

This standard was evaluated during the investigation and was found to be deficient. Two infants had similar looking bottles labeled with their names. A caregiver did not use good judgement when they did not check the name on one bottle before giving it to an infant. This resulted in the infant being fed a bottle, containing breastmilk, which belonged to the other infant.

A caregiver was present in a classroom with children without having a record of a background check being run for them under the operation's name. This was corrected at inspection when the caregiver left the operation, and the administrative staff ran a background check for the caregiver.

Based on information obtained during a DFPS investigation it was determined that due to a child being left unattended on the playground. The caregiver was not providing supervision that ensured the safety of the child in care.

Based on information obtained during the DFPS investigation it was determined the operation did not report that a child was left unsupervised on the playground.

Based on the information obtained during a DFPS investigation it was determined that parents of an child in care were not notified when that child was left unsupervised on the playground.

Based on information obtained during a DFPS investigation it was determined that caregivers at the operation were not redirecting children with positive statements. It was mentioned that spraying water from bottles was utilized to redirect the children.

At the time of inspection, one child did not have their completed vision and hearing screening on file for review.

As a result of the investigation, it was determined that the operation was deficient according to this standard. The caregiver was no aware of where the child was and stated so in their interview.

The operation did not complete the required survey to indicate the number of employees that left the operation during the year 2022.

During the inspection the operation allowed a staff member to be present without an initial background check being run. The operation corrected this during the inspection when the staff member left the operation during lunch and was asked not to return.

This standard was evaluated during the investigation and was found to be deficient. A caregiver left a child alone in the classroom. The child was found later by another staff member.

Based on a review of the parent signature in the child file and interviews conducted, it was determined the signature on the incident report was not the parent's original signature.

A caregiver was present in the childcare center in direct care with a group of children with a pending background check. Additional steps had not been taken to ensure clearance prior to the caregiver being present.

This standard was tasked over for this investigation and found to be deficient. During this investigation it was determined that a caregiver did hit a child with their hand while in care.

At the time of inspection, the operation did not have it's most current safety drills practiced on file for review.

At the time of inspection an infant child was observed asleep in a restrictive rocker chair. The child was removed by one of the caregivers present after being instructed to do so during this inspection time.

At the time of inspection, one child did not have his most current immunization record on file for review.

At the time of inspection, the drivers for the operation did not have a copy of their most recent transportation training certificate on file for review.

This standard was tasked over for the investigation and found to be deficient. The caregiver involved in this incident admitted to hitting a child in care. The caregiver named is no longer present at the facility and more redirection and behavior methods have been put in place to better assist the child in care.
