The infant room was observed with 21 children. The youngest child was 7 months and the oldest was 4 years old. The operation had more than 12 children in care. *3 children ages 7m, 9m and 18m remained in the infant room with 1 caregiver. All other children were moved to the pre-k room with 2 caregivers*

A caregiver providing care for children 24 months and younger is missing training in SIDS, Brain Development and SBS.

1 out of 6 childrens records reviewed is missing immunizations records.

The hallway leading to a classroom has a loose closet door that presents a safety concern in the event of a fall.

Five caregivers did not have the following required annual training topics: (1)Emergency preparedness (2)Preventing and controlling the spread of communicable diseases, including immunizations (3)Administering medication (4)Preventing and responding to emergencies due to food or an allergic reaction (5)Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6)Handling, storing, and disposing of hazardous materials

The infant daily report was missing the following sections: Mood and Activities.

An infant was observed asleep on a bouncer. Note: The infant was later placed in their assigned crib/play pen.

The monthly fire drills have not been documented.

Director did not have the 3 hours of training on management.

There was one caregiver caring for 5 infants. The specified age for the group was 0-11 months which allows 4 infants to 1 caregiver. Note The ratios were left in compliance as an additional caregiver was put in the classroom.

There was a loose section of the bottom moulding trim in the infant room which created a small opening on the wall. There were water stains on the ceiling on one of the classrooms used by the children.

Training certificates were not available for review.

Two translated education documents for two caregivers did not indicate the equivalency of education in the U.S.A.

There was trashcan without a lid in a classroom and the trash was accessible to the children in care. There were 4 tricycles with chipped paint and other bicycles with missing the rubber handle grips. There were purses accessible to the children in the classroom. Note: Purses were moved out of reach of the children.

Five caregivers did not have a current CPR/First Aid training.

The foreign education document translated in English for two caregivers did not provide supporting information to indicate the equivalency of a high school program.
