At arrival, two children ages 3yo and 4yo were sited on booster seats with straps. Note: Children were removed from the seats and transfered to regular chair without straps.

Operation has a play-structure with a dual glider swing.

Operation has an activity plan and daily schedule posted however, children were not observed participating in planned activities nor following the daily schedule.

The children's restroom cabinet was unlock and it stores personal items and cleaning products. Note: This was correction at inspection when provider locked the cabinet. Garbage including cups, bottles and food wrappers were observed at the operation's playground.

Play structure at operation has chipping paint, broke wood parts and tears at the base of the slides. There is an air conditioner at the operation's playground which is accessible to children.
