Roosevelt ISD Child Care Center

Three children were seated at a table in bucket seats but were not fastened in with the safety straps.

There were no in person training hours on file for CCR to review during the inspection for the director.

There were bird feces on slides that children had access to on the playgrond that had not been cleaned.

Preventing and responding to emergencies due to food or an allergic reaction and Administering medication trainings have been provided, however the director did not submit proof of completing annual training for Handling, storing, and disposing of hazardous materials, Wear gloves when handling blood or bodily fluids containing blood by the required compliance date of 9/18/2025.

It was found during the inspection that the director did not have proof of completing annual Shaken Baby Syndrome/Abusive Head Trauma, SIDS and Childhood Brain Development training since 2022.

It was found during the inspection that two caregivers were allowed unsupervised access to children prior to verification of completing orientation.

It was found during the inspection that the director did not have proof of completing annual training for Handling, storing, and disposing of hazardous materials, Wear gloves when handling blood or bodily fluids containing blood, Preventing and responding to emergencies due to food or an allergic reaction and Administering medication since 2022.

It was found during the investigation that an employee used prohibited discipline on a child in care by hitting the child.

It was found during the inspection that the last fire drill was documented 2/24/2025.

During the inspection, a blanket was observed in 2 separate play yards covering a child under 12 months of age. This was corrected during the inspection when staff removed the blanket from the crib.

During the inspection 2 infants were observed sleeping in a restrictive device. This was corrected during the inspection when the children were removed from the restrictive device.

It was found during the inspection that a caregiver was using their personal cellular device for personal reasons while caring for children.

It was found during the investigation that a caregiver was sleeping while caring for children.

It was found during the investigation that the director had knowledge of several incidents that placed children at risk and did not ensure that it was reported to Child Care Regulation.

It was found during the investigation that a staff member of the operation used inappropriate discipline on their biological child in while in care.

It was found during the investigation that employees used inappropriate discipline on children in care.

It was found during the investigation that the operation did not report a confirmed incident of inappropriate discipline used on a child in care to Child Care Regulation timely as required.

It was found during the inspection that the previous two fire drills exit times were more than 3 minutes.

While reviewing the people list in class, the operation last validated their employee list on 05/09/2022 and this is required to occur every 3 months. Correctedd at inspection as director validtaed the operations current employee list in the online portal.

The fire drill is not current as the last one was conducted on 09/23/2022. Corrected at inspection as a fire drill was conducted during inspection.

During the inspection it was observed that the operation did not have a current employee list posted as required. Corrected at inspection as the director typed up, printed nd posted the current employee list.
