The active play equipment used by children in care had less than 6 inhes of loose fill in the use zone.

During the inspection an employee was observed using inappropriate discipline on a child in care by grabbing and pulling on the child.

During the inspection an employee was observed changing a child s diaper on the floor area where other children were playing.

It was found during the investigation that caregivers left children alone inside of a classroom while the children were napping.

It was found during the inspection the operation does not have a plan in place to move the children to a designated safe area or alternate shelter if they have to exit through the playground area. The plan the operation currently has in place to exit from the playground puts the children in an enclosed area where they would be trapped in this area and have no way to get out of the area.

It was found during the inspection a caregiver stepped out of the classroom leaving the other caregiver in the room out of ratio.

It was found while reviewing records that one caregiver did not have the required hands-on skill-based instruction and practice CPR training.

It was found during the inspection the operations last fire inspection was on 4/21/23 therefore it has been over a year since their last fire inspection.

It was found during the investigation where an infant was placed in a classroom with the oldest child in the group being 23 months older than the infant.

It was found during the investigation an employee was allowed to be present at the operation and have access to the children at the operation before receiving the employee's notice from the Criminal Background Check Unit regarding the employee's eligibility.
