One classroom was observed eating snack on the floor of the classroom.

While transitioning from paper to electronic, the operation misplaced personnel records and were unavailable for review during inspection.

Although the operation created an incident report via ProCare, the operation did not complete incident report form 7239 which is provided by CCR and requires parent signature.

One child did not have an immunization record in their file.

Four children did not have a physician statement stating the child was well enough to attend care.

Two children with diagnosed food allergies did not have all of the medication listed on their food allergy plan present at the operation.

The diaper changing pad in the infant room had tears exposing absorbent material that prevents it from being properly sanitized. A new diaper changing was ordered during the inspection.

One infant did not have infant feeding instructions available for review.

Information obtained during a DFPS investigation shows that a television is used in the infant room.

During a DFPS investigation there was sufficient evidence to support that an infant was able to climb out of a highchair and hit their head on the floor. One caregiver walked away from the infant sitting in the highchair, had their back towards the infant, and was using their personal cell phone. The other caregiver was facing the infant while sitting on the carpet and failed to get up and stand in front of the highchair knowing the infant has attempted to get out of the highchair previously.

During a DFPS investigation, there was sufficient evidence to support that caregivers neglected a child in care.

Information obtained throughout the investigation shows that caregivers use their personal cell phones while supervising children.

Information obtained during a DFPS investigation shows that a caregiver failed to properly fasten the safety straps on a highchair as the safety straps were fastened, but not tightened resulting in an infant climbing out of the highchair and falling to the floor, hitting their head.

Observed the fire inspection had not been completed since May 2020. I spoke to the director about ensuring the inspection is scheduled each year.

Out of 10 children records reviewed, 3 children were missing health statements. I spoke to the director about the health statements.
