The caregivers in Elm I and Maples I did not know the correct number of children they were caring for.

6 of the 10 caregivers' files were reviewed during the inspection; CPR courses did not adhere to guidelines.

The operation has three children with identified allergies on their admission form that do not have a food allergy plan on file.

Based on information reviewed during the inspection, the operation does not have documentation that a carbon monoxide system is installed at the operation and has been tested.

The operation trash bins do not have libs.

The operation has a foster care child that has been in care since May of 2026 with no immunization records on file. CI: The child's parent emailed their immunization records during pick up.

Based on information gathered during the course of the investigation, it was determined that a caregiver was supervising children while using a cell phone device for personal reasons.

Based on information gathered during the course of the investigation, the director was aware of a situation that placed children at risk and did not report it to CCR.

Information gathered during the investigation indicates that the operation did not provide appropriate supervision to children in care. A caregiver walked out of the classroom and left children alone during naptime.

It was determined that a caregiver slapped a child in the face.

Staff are missing the following trainings; -Four staff are missing recognizing and preventing shaken baby syndrome and abusive head trauma, -Two staff are missing understanding and using safe sleep practices and preventing sudden infant death syndrome and -Four staff are missing understanding early childhood brain development.

Staff files were observed missing the following; - One staff is missing training in emergency preparedness; -Six staff are missing training in preventing and controlling the spread of communicable diseases, -Three staff are missing training in administering medication, -Six staff are missing training in preventing and responding to emergencies due to food or an allergic reaction, -Six staff are missing training in understanding building and physical premises safety, and -Six staff are missing training in handling, storing, and disposing of hazardous materials

A caregiver did not properly position themselves on the playground when a child was able to open the emergency gate and get out. The caregiver was unaware the child was outside of the gate.

A parent was not notified immediately of a situation involving their child.

Eight staff files were observed and missing the following; Two staff are missing a signed statement that they received the center policies. Two staff are missing a signed statement that they received the personnel policies. Two staff are missing signed statements for completing orientation.

Ten children files were observed and missing the following; Five children files are missing the special care statements. Three children files are missing the parents signing off on receiving the parent handbook. Three children files are missing health statements.
