During the May 12, 2026 , inspection, there were 16 three year old children in attendance in the Explorers 1 classroom with only one caregiver. This is one more child too many for only one caregiver to supervise with this age group according to minimum standards. The operation brought this deficiency into compliance during inspection by adding an additional caregiver to the classroom.

The operation was found to have exceeded the maximum number of children allowed in a classroom with a single caregiver based on the classroom's specified age group.

At time of inspection, one out of the two director files did not have training certificates for 2024-2025 avaliable for review.

At time of inspection, one caregiver conducted CPR training online.

At time of inpsection, one director had 29 hours of annual training for 2024-2025.

A plate size portion of the unitary surfacing on playground was observed missing.

During inspection, two out of the six alarm did not sound.

There was no admission and withdrawal date on a child's admission form. This was corrected at inspection when director wrote down the date date for admission and withdrawal.

Child were observed playing video games in the school age classroom. This was corrected when director turned off the tv.

Based on evidence received during the DFPS investigation, a caregiver used poor judgement by moving a bottle warmer from the counter to the floor, within reach of the infants in the classroom. This poor judgement call placed the infants in the classroom at risk, and a child was burned.

Based on evidence received during the DFPS investigation, a caregiver moved a bottle warmer from the counter to the floor, plugged it in, and walked away. The caregiver?s neglectful behavior resulted in a 11 month old infant to be left unsupervised with a hot bottle warmer and the infant sustained first and second degree burns on both legs.
