A 5 month old infant was observed to be in the same room as a two year old child.

A 5 month old infant was observed to be sleeping in a playpen with a blanket covering the waist and legs. Note: The blanket was removed immediately.

The toilets were observed to be dirty and in need of maintenance.

The written activity plans for five classes did not have the group name and date.

Based on the information obtained during the course of the investigation, a child walked out of the operation and was left unsupervised.

A caregiver was observed to be supervising children alone without a pediatric first aid with rescue breathing training.

One out of ten children records evaluated did not have an immunization record on file. Two out of ten children records evaluated did not have an updated immunization record on file.

The toilets in the purple, orange, and green rooms were observed to be dirty and in need of maintenance. The garbage cans in the restrooms were observed to not have a lid making the garbage accessible to children.

Three out of ten employee records evaluated did not have the following trainings: hazardous materials, allergic reactions, administering medication, emergency preparedness, building safety, and communicable diseases.

Ten employee files were reviewed. Six files were pending background check request letter. One file was pending the education requirement and picture ID.

Children's products certification form was not available for review.

Ten children's files were reviewed. Seven children's files were pending updated immunization records.

Use zone areas for climbing and bouncing equipment and slides do not have the required 6 feet of surfacing.

Observed infants and caregivers not wash their hands after outdoor play.

Infants and toddlers were served mixed fruit, including whole grapes.

There were three buckets of water in the hallway outside of the classrooms. Buckets were unattended. Note: Buckets were removed from hallway during inspection.

Ten employee files were reviewed. Seven files were missing the cpr/first aid certificates.

Annual training was provided by Director. Operation received an administrative penalty within the last two years.

Posted activity plans did not include the name of the class. It did not list specific daily activities. Outdoor activities were not listed.

Three trash cans in restrooms used by children were observed to be uncovered. Garbage was accessible to children. Note: it was corrected during inspection.

Ten children's records were evaluated during inspection. Seven records were missing the health statement.

Ten children's files were reviewed. Four files were missing immunization records.

Operational polices did not include the following information: Promotion of indoor and outdoor activities; How the parent may access the minimum standards online; Instructions on how a parent may access the HHSC website; Center's emergency preparedness plan; and information on preventing and responding to abuse and neglect of children.

There is one sink in the infants' room. Sink is being used for purposes other than washing hands.
