Two infants were found to have expired written feeding instructions.

Cleaning supplies were stored unsecured under the hand washing sinks in two bathrooms located in the Family Life Building used by children.

During an investigation, it was determined that a child was left in a bathroom unsupervised for approximately three minutes until found.

The operational policies do not meet minimum standards.

A child subsequently gained access to the knife and sustained a minor cut.

One out of the ten reviewed children files did not have the updated and required information for the food allergy emergency plan.

One out of the medications inspected was expired on July 2024.

Gas was last inspected on 01/29/2020.

There was approximately one inch of loose fill at the end of the slide. This was corrected at inspection when a staff member distributed the loose fill to the necessary spot to maintain the 6 inch loose fill requirement.

Medications were observed to have expired for children no longer enrolled.

Training for SIDS for 2023-2024 was unavailable for review.

A child was left unattended in a restroom down the hallway from the classroom. The caregivers did not conduct a name to face count prior to leaving the restroom or upon arriving at their assigned classroom.

Based on the information provided, a child was wandering the hallway unsupervised.

A child was left in a classroom unsupervised; line of sight was lost when the door was closed, and class was in the restroom.

A caregiver did not double check to ensure she had all the children after leaving the bathroom and one child was left in the bathroom. Two additional caregivers walked into the bathroom; however, they were not responsible for this child and were not aware that the child was in the bathroom.

Two children were left unsupervised in the restroom.

A child was left unsupervised by a caregiver for one minute during naptime.

The Child Product Safety Commission form available was dated 09/06/2020. This is outside of the required annual update. This was corrected at inspection when the director updated and posted a new Child Product Safety Commission form which was dated 09/09/2022.

Garbage can in the three year old classroom was missing it lid. Lid was broken and garbage was accessible to children. This was corrected at inspection when director replaced garbage can with a working lid.

During of the course of the investigation it was proven a four year old child was left unattended in classroom for two minutes.

A sixteen month old left was unsupervised in the hallway about one minute

A child exited the playground gate without supervision or assistance from the caregiver after seeing parent at the operation.
