Based on the information obtained during the investigation, it was determined that a caregiver allowed an infant to fall asleep in a restrictive device (swing) for approximately twelve minutes before removing the infant from the device and placing the infant in a crib.

Two caregivers counted in child/caregiver ratio did not have the required eight hours of pre-service training.

5 Of 6 personnel records reviewed were missing the a Pre-Employment Affidavit for Applicants for Employment at Certain Child Care Operations (Form 2912).

A caregiver was missing annual training for training year 7/7/2024 - 7/6/2025 on the following topics: (1) Prevention, recognition, and reporting of child maltreatment; (2) Recognizing and preventing shaken baby syndrome and abusive head trauma; (3) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); (4) Understanding early childhood brain development; (5) Preventing and controlling the spread of communicable diseases, including immunizations; (6) Administering medication; (7) Preventing and responding to emergencies due to food or an allergic reaction; and (8) Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

Two caregivers who have unsupervised access to the group of children do not have a CPR training.

The vent in a classroom was observed with dust and debris. Corrected at Inspection: The vent was cleaned.

Three caregivers conducted an online CPR training.

Loose fill surfacing (mulch) was not observed to be maintained at the proper depth for a play structure in the outdoor play area. The dirt underneath the mulch was visible. Corrected at Inspection: The structures were removed.

A Food Allergy Emergency Plan for a child was not signed by the parent and health care professional.
