The director did not have 30 hours for the most recently completed training year.

The operation was missing ID's and proof of education for two caregivers. They were missing a notarized affidavit for one caregiver and the pre-employment affidavit for another caregiver. One caregiver did not have her annual trainings available during inspection.

It was found during the DFPS investigation that caregivers are putting children on the porch and in the kitchen for discipline while the caregivers remained in another room. It was also found that caregivers let an 8 year old walk around the center unsupervised by staff.

The operation did not have a current health inspection.

The operation hasn't done recent monthly playground maintenance checklists. This was corrected during the inspection.

The posted emergency diagrams didn't show the designated location inside the child care center where all caregivers and children take shelter from threatening weather.

The operation didn't have recent documentation of doing emergency drills or checking fire extinguishers and smoke detectors.

Two caregivers were missing annual training on administering medications, allergy emergencies, and handling hazardous materials.

The operation did not have a current fire inspection.
