The diaper changing mat in the toddler room was observed with rips and tares. This was corrected at the inspection when the director replaced the mat with a new one.

The caregiver did not wash the child's hands after changing the child's diaper. This was corrected at the inspection when the caregiver was observed doing diaper changes and washing the children hands.

4 cribs were observed with mattresses that did not fit firmly against the crib. This was corrected during the inspection when the caregiver changed the sheets on the mattresses to allow for a firm fit.

The operation did not notify the parent of the child of an incident that occurred at the operation.

During the inspection one child were observed climbing on two different book shelves, while two others were observed climbing into shelves with doors and openings.

A caregiver was observed with screen time activities during a rest period. This was corrected when the caregiver turned the screen off.

One caregiver were observed not washing hands after diapering a child, and anther after placing hands inside a garbage can. This was corrected when the caregiver washed their hands and that of the child.

There were diaper creams stored in an unlocked cabinet underneath the diaper changing station where children could reach them. This was corrected during the inspection when the creams were moved to a cabinet out of the reach of children.

During the walk-through cribs were observed with loose fitted sheets and mattresses that were smaller than the manufacturer's design. The sheets and mattresses were adjusted to fit the cribs.

Children were observed watching television and cell phone screens during a designated nap or rest period.

Children were observed sleeping on the floor and on torn sponge mats with blankets.

During the walk through, toilets in the classrooms had not been flushed. The porcelain backing of the toilet was laid across a shelf and accessible to the children posing a potential risk. This was corrected at inspection when the toilets were flushed and the porcelain back was place on the toilet.

During the walk through, the operation was not following the conditions of the CBCU with regards to background checks.

During the insepction children were observed watching televsion while other chidlren were allowed to nap.

Children in the toddler classroom were observed napping in one corner of the room where all of their mats or cots were touching and combined resulting in no walk space between the children. This was corrected at inspection when children were separated to allow for adequate spacing.

There was an outlet that was uncovered and accessible to children in an infant classroom and a toddler classroom. This was corrected at inspection when both outlets were covered using a safety cover.
